Paper Order Form (If completing paper form, order must be mailed with check to the address on the form.  Orders will NOT be accepted in the Urbandale Activities Office.)

**Your letter jacket will not have your academic/athletic letter attached.  Upon receipt of your letter, you'll need to make arrangements with a local seamstress to sew your letter to your jacket.
